Women having facial hair does not mean they're transitioning. It may just mean they have facial hair. Even if in this case it's from PED use.

The main distinction between a male and a female are the genitalia and presence of a uterus.
Testosterone and estrogen hormones are a bit different, as both genders have it, but the male has an exponentially higher amount of testosterone and very less estrogen, and the reverse is visible in the female.
Here, when the female transition into a male, it doesn't mean that they've gained a male genitalia or something. They do take in testosterone through medical means (prescription drugs or surgery). Their birth genitalia is also altered to some extent.
Most of the time, the female who is transitioning already had a higher amount of testosterone compared to the existing women, and as such felt their identity more inclined towards the male gender.
I think the explanation would not make a 5-year-old understand this, but oh well, it's not something kids should be taught at such an early age, haha. Hope it helped in some manner.
